To ensure the best treatment for the pet by sharing medical data between clinics, I hereby grant

the following consent
The Clinic has the right, in case of a treatment event of the pet(s) belonging to me (hereinafter: the Pet(s)) for the purpose of providing veterinary consultations and procedures:make available all of the information about the Pet (including previous medical history and entries related to veterinary services, excluding the invoices issued) along with my personal data (in particular name, personal identification code, document number, residential address, telephone number and e-mail address) which is included in the customer management software for veterinary clinics named PetBook (hereinafter: PetBook) and used by the Clinic, immediately via PetBook software application when another clinic using PetBook or its veterinarian (hereinafter: PetBooki kliinik), on an as-needed basis (i.e., if I have gone to that other clinic with my Pet to receive veterinary services), makes such access request to the Clinic for the purpose of providing veterinary services to the Pet.
The PetBook Clinics have the right to further process the data shared in accordance with the above for the purpose of providing veterinary services and ensuring the best treatment for the Pet in accordance with the principle of data cross-use (i.e., if one PetBook Clinic makes a new entry regarding the Pet’s medical history in PetBook or modifies/updates my and/or the Pet’s data there, it will immediately become visible and usable to other clinics with which the data exchange has occurred and is permitted in the described manner) in accordance with the privacy policies of the corresponding clinics. Every time I go to a PetBook Clinic, I agree to review their privacy policies (which are available on the corresponding clinic’s website or at the clinic itself). I am aware that the PetBook Clinics cross-using my personal data in accordance with the foregoing are considered as joint-controllers with respect to the relevant processing of my personal data and I have the right to exercise my rights in relation to my personal data by contacting any PetBook Clinic with which my personal data is shared and cross-used in accordance with this consent.
This consent is sufficient for the purposes of the transmission and further cross-use of data described above, without the need for any further separate written consent to that effect and is given for an unspecified term. This consent also applies to any previous data processing that has already taken place between PetBook Clinics at the time of signing for the purposes described in the consent.

transmit all information about the Pet in PetBook (including previous medical history and entries related to veterinary services, excluding invoices issued) along with my personal data to the extent necessary for identification (in particular, name and personal identification code) immediately, if the other veterinary clinic (or its veterinarian), on an as-needed basis (i.e., if I have contacted this clinic and/or veterinarian with the Pet for obtaining veterinary services), contacts the Clinic with the corresponding request. This consent is sufficient for the purposes of the transmission of data described above, without the need for any further separate written consent to that effect and is given for an unspecified term. I am aware that in such a case the veterinary clinic that is the recipient of the data is a separate controller of my personal data (i.e., the Clinic is not responsible for further processing of the data), and I undertake to read the privacy policy of the corresponding clinic.
