CLIENT AGREEMENT TO SERVICE PROVIDER

To ensure the best treatment for my pet(s) when visiting different veterinary clinics by sharing data between clinics, I hereby give my consent to the PetBook Service Provider (as defined in the Terms of Use of the Petbook Database Service) to process my personal data.

The PetBook Service Provider* (VetMinds Invest OÜ, registry code 14644037, address Mustamäe tee 44a, 10621 Tallinn, e-mail info@petbook.ee) has the right to:

  1. make available, through the PetBook software application, all information about the Pet included in PetBook (including previous medical history and entries related to veterinary services, excluding the invoices issued to the User by a clinic) along with the User’s personal data (in particular name, personal identification code, document number, residential address, telephone number and email address), immediately when another PetBook Clinic (or its veterinarian), on an as-needed basis (i.e. the User has gone to that clinic with their Pet to receive veterinary services), makes such access request to the Service Provider for the purpose of providing veterinary services to the User’s Pet.

    The PetBook Clinics have the right to further process the data shared in accordance with the above for the purpose of providing veterinary services and ensuring the best treatment for the Pet, in accordance with the privacy policies of the respective clinics, and the Service Provider has the right to cross-use the data for the purpose of providing the Service, i.e., if a PetBook Clinic makes a new entry in PetBook about the medical history of the User’s Pet or changes/updates the details of the User and/or the Pet in PetBook, this information will immediately become visible to and usable by the other PetBook Clinics with whom the data exchange has taken place and is permitted in the described manner, and to the Service User via the PetBook user account; and vice versa, if the Service User updates his/her data in PetBook, it will immediately become visible and usable in the PetBook application for other PetBook Clinics with whom the data exchange has taken place and where cross-use in the described manner and for the described purposes is permitted.

    This consent is sufficient for the purposes of the transmission and further cross-use of data described above, without the need for any further separate written consent to that effect and is given for an unspecified term. This consent also applies to any previous data processing that has already taken place between PetBook Clinics at the time of signing for the purposes described in the consent.

    Every time the User visits a PetBook Clinic, the User undertakes to read the privacy policy of that clinic (which is available on the website of the corresponding clinic or at the clinic itself) and the User is aware that the PetBook clinics and the Service Provider, who cross-use the data, are considered as joint-controllers in respect of the personal data processing carried out accordingly, and that the User has the right to exercise his/her rights in relation to their personal data by contacting both the Service Provider and each PetBook Clinic with which the User’s personal data has been shared and will be cross-used in accordance with the aforementioned consents;

    and
  2. transmit all information about the Pet in PetBook (including previous medical history and entries related to veterinary services, excluding invoices issued by clinics to the User) along with the User’s personal data to the extent necessary for the identification of the User (in particular, name and personal identification code) immediately, if the other veterinary service clinic (or its veterinarian), on an as-needed basis (i.e., the User has contacted the corresponding clinic and/or veterinarian with the Pet for obtaining veterinary services), contacts the Service Provider with the corresponding request. In this case, this consent is sufficient for the transmission of the described data without the separate written consent of the User and is given for an unspecified term. The User is aware that in such a case the veterinary clinic that is the recipient of the data is a separate controller of the User’s personal data (i.e., the Service Provider is not responsible for further processing of the data), and the User undertakes to read the privacy policy of the corresponding clinic.

By giving this consent, I confirm the following:

  • it has been explained to me which clinics are currently or have previously been using PetBook, including Loomaarst OÜ or Keila Veterinary Clinic, Osaühing Maxi loomakliinik and Evidensia Loomakliinikud OÜ (formerly Loomade Kiirabikliinik OÜ, previously as separate legal entities Loomade Kiirabikliinik OÜ, Timmu Loomakliinik OÜ and OÜ Rakvere loomakliinik);
  • I am aware that I have the right to withdraw this consent at any time* by submitting a request to that effect to the above email address of the Service Provider, and that I may, with regard to the processing of my personal data, file a complaint to the Service Provider or the Data Protection Inspectorate (website: www.aki.ee) if I find that the processing of my personal data violates my rights and interests.
